What to Bring to Your First Appointment

If possible, all new patients should bring the following to their first appointment:

  1. All medications you are currently taking;
  2. Recent MRI reports;
  3. Drivers License;
  4. Insurance Card.

New Patient Forms

Our new patients should be prepared to fill out our patient intake form. This form can be downloaded from this page or filled out the day of your appointment. The new patient form includes sections on general information, medical history, as well as a narcotic agreement.

Physician Referrals

Our goal is to provide our referring physicians and their practices with a straightforward patient referral process that takes only a few minutes to complete. Below you can download our fax referral form. Simply fill out the form, fax it to (770) 771-6589, and we will call the patient to schedule their appointment.
